Community Supports Program

The Community Supports Program (CSP) is based on the belief that individuals have a right to live in the community and have access to required supportive services.
The services provided under the CSP focus on supporting individuals, families and care providers by promoting independence, community inclusion, safety and overall well-being, in keeping with Eastern Health’s vision of “Healthy People, Healthy Communities.”

Services provided under the CSP include:

Referrals to the CSP can be made by the individual or their family, as well as a health care professional. The professional CSP team will determine eligibility for service.

How to Qualify
The Community Supports Program provides supports to individuals who meet one of the following criteria:
  • Are at least 18 years of age and older who have physical and/or intellectual disabilities, requiring long term assistance with daily living and/or supplies and equipment
  • Are under 18 years of age requiring Special Child Welfare Allowance
  • Are at least 65 years of age and over requiring assistance with daily living and/or supplies and equipment which would enable them to remain at home
  • Require protection under the Adult Protection Act
  • Meet the financial eligibility criteria of the Community Supports Program
  • Are children of school entry age and older who have an intellectual disability and significant behavioural concerns
  • Are children 18 months of age and older who experience assessed developmental delays, or who are diagnosed with Austism Spectrum Disorder
